Health Examination Form - NEW!
The following form must be completed and returned to us before the start of camp:
The Health Examination form must be signed and dated by both a parent/guardian AND your child's physician. Forms are valid for one year only, and must be valid through the last day your child plans to attend camp. Campers will not be able to participate without a completed medical form on file. Please note, the city has recently updated their Health Examination form for 2011. If you have already submitted an up-to-date form in the old format, you do not need to re-submit your form.
