NO, you do not! The link to the NYC Physical Examination form is provided as an optional sample form, as some families and doctors request a template. We do not require this exact form. We will happily accept the form provided by your camper's doctor's office, as long it is is a physical within 1 year and immunization records

The plan for our youth summer camps is to be open for campers every day. We have the ability to access indoor facilities and accommodate campers should inclement weather limit playing on the fields. For the safety of the campers we also use our lightning-detection system that triggers sirens and our staff gets everyone into facilities.
Please let us know at registration or on the medical forms if your child has any allergies. We will make the trainer on staff aware as well as any one working the concession stands. If you child has an Epi pen or any other forms of medication please make sure to drop it off at the beginning of the day in a zip lock bag labeled with your child’s name. All medications must be picked up at the end of the day at check out.
Our staff will make sure each coach knows who may have allergies. We also make every attempt to wash hands and protect the camper.
